Medical Visa from Pakistan to Belgium Description

  • A medical visa is issued to a patient who needs medical care in a hospital or clinic in Belgium.
  • Belgium visa application form should be completely filled, printed and signed by applicants.
  • Original Pakistan passport that is valid for at least 3 months beyond stay in Belgium.
  • The passport must have at least two blank pages Belgium visa stamp.
  • Photocopies of the applicant’s previous medical visa are required.
  • Two identical passport photographs displaying a neutral expression.
  • A local medical report issued by a doctor/clinic/hospital in Pakistan that the applicant needs medical treatment is required.
  • A medical attestation from the hospital or doctor in Belgium, confirming the dates of the applicant’s appointment as well as a medical situation.
  • Receipt showing proof that the applicant has paid the medical fee.
  • Official confirmation of the treatment by the receiving medical institution in Belgium.
  • Proof that applicants have sufficient funds to pay for the medical expenses and other related expenses throughout treatment.

Medical Visa from Pakistan to Belgium Requirements

  • Belgium visa application form that is fully completed, printed, and signed at the end.
  • Two identical passport photographs are required.
  • The photographs should be no older than three months and taken must be taken against a white background.
  • The Pakistani passport should contain at least to blank pages in order to be able to affix the Belgium visa sticker. 
  • The passport must be valid for at least 3 months beyond the date you exit Belgium.
  • Applicants are required to provide copies of the previous visas and passports if available.
  • Applicants are required to provide all documents either translated in either English, French, German or Dutch.
  • Travel/medical Insurance for Belgium with a minimum of €30,000 coverage for medical emergencies, and repatriation in case of death.
  • Round-trip/flight itinerary containing the dates and flight numbers specifying entry and exit from Belgium.
  • A document that shows where you will stay in Belgium. I.e. Hotel reservation for the whole duration of the intended stay in Belgium.
  • Proof of civil status. This could be a marriage certificate, birth certificate of children, or death certificate of spouse.
  • Proof of sufficient financial means for the period of stay in Belgium. According to the Belgian Immigration Office, a traveler has to attest possessing €95 per day if staying in a hotel, and €45 when planning to get accommodated by cheaper means.
  • Minors applying for the Belgian visa are required to submit a copy of the birth certificate, a notarized parental authorization to travel to Belgium, signed by both parents / guardians if the minor will be travelling alone and certified copies of ID / passport of both parents.
  • Belgium application form for minors are required to be signed by both parents.
